The primary role of the RCC is to provide chronic kidney care coordination. The RCC works with the patient and members of the healthcare team with a focus on delaying the progression of CKD. The RCC also educates patients on options for renal replacement therapy (RRT), helps patients select appropriate RRT and assists with pre-emptive renal transplant and dialysis access planning.
Another important function of the RCC is to maintain close contact with patients. Patients are primary members of the health care team. It is essential to communicate any issues or concerns with members of the team so that together we can focus on keeping patients healthy and most importantly avoiding hospitalizations.
