The Kidney Care Team is a group of health care professionals whose job is to make you successful at managing all aspects of your health. The members of the team include your doctor, a clinical pharmacist, a nurse care manager, a physician assistant, a dialysis educator, and most importantly, YOU. The team will work to help you navigate through all aspects of your chronic kidney disease. They are here to answer your questions regarding disease progression, diet and exercise, medications and any other concerns you may have.
We at Kidney Care understand these challenges and the importance of helping you to succeed in managing your health, in the real world – your real world.
